How to Use This Editable PDF

  1. You will need Adobe Acrobat Reader software to open the file. You can download it for free here.
  2. Click on the link above to open the PDF. Please note, you will not be able to save a version of the file that contains your changes. However, you can save a copy of the template itself, which will let you open and modify it again so you can do your work in several sessions.
  3. To change the words, highlight the portion of the text and retype your specific information.
  4. Because you cannot save an online copy of what you type, proofread carefully on the screen before printing it out.
